In future, we will be transitioning other PDF Forms to Online Forms. Online Forms will only be available in the LSLBC Contractor Portal.
Portal Instructions: Once logged in to the Portal as the Applicant or Licensee, you will click “View” next to the Credential #, and you will see “Forms” option on the left side. The “Forms” option will not appear if viewing a Qualifying Party’s information.
